Affordable Care Through Our Sliding Fee Scale

AMH’s Sliding Fee Scale (SFS) is a discount program based on household size and income. It can help patients without insurance, as well as insured patients who need help with out-of-pocket costs.

  • Anyone can apply, qualification is dependent on your household and income shown here.

    • You can download an application HERE or ask the front desk for an application at any AMH location

    • List your household members and total income and return the form for a temporary 30-day discount.

    • Submit proof of income within 30 days to keep your discount.

    • Once approved, your SFS eligibility is valid for 365 days and must be renewed annually.

    • Recent pay stubs

    • Tax return or W-2

    • Social Security or disability benefits letter

    • Unemployment or workers' compensation letter

    • Pension or retirement statement

    • Other official income documentation

  • Only the people who live in your residence and whom you financially support or who financially support you.

  • Yes, if they support you financially.

  • Your discount ends, and regular rates apply.

  • Yes. We bill your insurance first. After insurance pays, the Sliding Fee Scale can help with high out-of-pocket costs, so you won't pay more than your approved SFS copay.
